Q. How many employees must my company have to qualify?
A. The Dental Benefit is available to groups of 3 or more employees. There is no limit on the number of employees.
Q. Which employees are eligible?
A. All full-time, permanent employees who work a minimum of 24 hours per week are covered under this program. Contract employees (category 1099 employees) are not eligible. If your group-sponsored medical plan uses a definition of at least 20 hours per week, the same definition may be used for the dental or vision plan.
Q. Is my company eligible?
A. Most industries do qualify, although some have been excluded for example, companies that hire seasonal employees, or have high turnover.
Q. What portion of the cost are employers required to contribute?
A. $0, employer is required however to provide payroll deduction for those who enroll, and remit the contribution using the reporting form provided by the administrative office.
Q. Does Voluntary Workplace product have a rate guarantee?
A. Yes, your rates are guaranteed for two years from the initial effective date for the employer. New hired employees will have their rates guaranteed until the end of the employers 2 year anniversary.
Q. What are the age limits for dependent children?
A. Dependent children are covered to age 19. If they are full-time students, their coverage is extended to age 25.
